چکیده
purpose: to report the outcomes of surgical and non-surgical treatment for sixth nerve palsy and paresis in our center. methods: hospital records of 33 patients (35 eyes) with sixth nerve dysfunction who were referred to labbafinejad medical center from september 1996 to september 2006 and underwent surgical procedures or botulinum toxin injection were reviewed. results: patients were divided into three groups: group a had muscle surgery without transposition; group b had transposition procedures and group c had botulinum toxin injection. the deviation improved from 50.3±16.8 to 6±9.8 prism diopter (pd) after the first operation and to 2.5±5 pd after the second operation in group a; from 56.9± 24.3 to 5.5±16 pd after the first operation and to almost zero following the second operation in group b; and from 44.3±10.5 to 15±20 pd 6 months following botulinum toxin injection. head posture and limitations of motility also improved significantly in all three groups. the overall reoperation rate was 18.2%. conclusion: various procedures are effective for sixth nerve dysfunction; all improve ocular deviation, head turn and abductive capacity of the eye. the rate of reoperation is not high if treatment is selected appro-priately according to the patient's condition.

Isolated sixth nerve palsy
An isolated sixth nerve palsy may be a harbinger of underlying intracranial disease. Due to its long subarachnoid course, it may be damaged by downward shift of the brainstem as often occurs in increased or decreased intracranial pressure (“false-localizing sixth nerve palsy”). Alternatively, the sixth nerve may be involved in isolation by a compressive lesion in the cavernous sinus or along th...

PURPOSE To report the outcomes of surgical and non-surgical treatment in sixth nerve paresis and palsy. METHODS This retrospective study was performed on hospital records of 33 consecutive patients (37 eyes) with sixth nerve dysfunction who were referred to Labbafinejad Medical Center from September 1996 to September 2006, and underwent surgical procedures or botulinum toxin injection. Patien...
متن کاملSixth nerve palsy of para-infectious origin.
Acute abducens palsy of para-infectious origin is uncommon in adulthood. We present the case history of a 28-year-old woman suffering from acute onset diplopia due to abducens palsy. Magnetic resonance imaging revealed signs suggestive of frank ipsilateral maxillary sinusitis. The lack of anatomical relationship between the maxillary sinus and the abducens nerve led us to conclude that the abdu...
متن کاملNasal cause of sixth cranial nerve palsy.
Anatomically the cavernous sinus is closely related to the sphenoid sinus and the nasopharynx. Sphenoid sinusitis, as demonstrated in the case can affect the nerves which are contained within it. Apart from the sinus, a direct extension from a nasopharyngeal lesion is commonly encountered to cause diplopia, owing to the sixth nerve involvement. The provisional diagnosis should be a nasopharynge...
متن کاملIsolated sixth nerve palsy from pontine infarct.
Isolated sixth nerve palsy associated with pontine infarct is very rare due to close anatomic organization of the structures. A 62-year-old woman, who complained of diplopia, had a diagnosis of sixth nerve palsy. Ophthalmological examination revealed 30 PD left esotropia in primary position with limited abduction of the left eye. Neurologic examination was normal. MR showed a lacunar infarct in...
متن کاملCapecitabine and sixth cranial nerve palsy.
Capecitabine is an oral chemotherapeutic agent converted to 5 fluorouracil (5-FU). Neurotoxicity associated with the medication encompasses both central and peripheral nervous systems. We describe a 60 year old man with colonic carcinoma who developed diplopia due to a sixth nerve palsy following the use of capecitabine which is an orally administered prodrug of 5-FU. An MRI of brain did not re...
